As such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

The Coordinator, Quality, Safety & Outcome Improvement reports to the Director(s), Quality, Safety and Accreditation and is actively involved in the planning, development, implementation and evaluation of quality improvement initiatives and maintains primary coordination of quality initiatives for the Director(s) and team with a person-centred approach related to quality and safety. This includes the broad areas of knowledge exchange, information dissemination, and education for internal and external stakeholders. This role is responsible for supporting the Director(s), Quality, Safety and Accreditation with accreditation readiness and sustainment including coordinating staff working groups, mock surveys, the Accreditation survey work plan, and on-site visits and supports the communication of results to leadership and relevant stakeholders. The Coordinator identifies, collects and analyzes information and metrics related to quality initiatives and patient safety reviews and incidents; reports findings regarding the implementation, sustainment and effectiveness; and provides related input and suggestions based on the data, best practices and current research to the Director(s). This role compiles the quality and safety action plan and an annual report, in collaboration with the Director(s) and program teams, to communicate and highlight quality improvement initiatives for the program.


What you’ll do

  • Coordinates the execution of quality initiatives that improve patient safety standards by drafting project charters, process maps, and timelines for the team using provided information; maintaining action logs; providing key stakeholders with status updates regarding quality initiatives; and following up with team members regarding meeting action items and tasks to confirm completion within the required timelines, adherence to project protocols, and conformance to applicable institutional and ethical policies and procedures and privacy legislation. May include supervision of staff or students assisting in large or complex quality initiatives.
  • Evaluates quality initiatives by identifying, collecting, and analyzing relevant information, including outcome and process metrics related to current quality initiatives and patient safety reviews and incidents to confirm implementation, sustainment and effectiveness. Reports on findings and provides input and suggestions based on the data, best practices and current research to the Director(s) and quality initiative/program team for development, implementation, analysis, dissemination of findings, and policy/practice implications.
  • Actively supports the Director(s) with the management and facilitation of time-limited quality initiatives and accreditation readiness and sustainment, including obtaining staff input and organizing working groups with relevant stakeholders as indicated by the Director(s) to collaboratively determine priorities, identify better practice strategies and activities for health professionals, and develop and implement policies and procedures to improve quality, safety and patient experience and meet Accreditation Canada and other accreditation agency standards as appropriate.
  • Coordinates and prepares for meetings through methods such as drafting agendas, ensuring reports and tracking tools are pre-circulated, records meeting minutes including the action items, associated responsible person and timelines for tasks, and progress is reported at subsequent discussions. Drafts and implements communication plans and compiles written reports on results to inform leadership and relevant stakeholders regarding quality initiatives.
  • In collaboration with the Quality Leaders and/or Clinical Leads, tracks the progress and completion of approved recommendations resulting from patient safety event reviews.
  • In collaboration with the Director(s) and program teams, compiles the quality and safety action plan and an annual report to highlight quality improvement initiatives for the program. Works with the Director(s) and Quality Leaders to develop, implement and analyze evaluative surveys and special projects to support the development of the quality and safety action plan and quality improvement initiatives.
  • Develops and maintains a database that conforms to accreditation standards and measures to track data such as recommendations related to patient safety reviews. Analyzes the data from patient safety reviews to identify data trends, such as common gaps across departments, to provide notification and suggestions to the Director(s) and quality improvement/program team.
  • Works in collaboration with the Director(s) to prepare for the Accreditation survey for the program by meeting with Accreditation Canada Surveyors to understand expectations, compiling documents to provide to the Surveyors, and developing a work plan, including drafting timelines and coordinating with the patient care areas to arrange on-site visits.
  • Coordinates mock surveys by scheduling on-site visits in the patient care areas and preparing the survey tool for the Director(s) and the Quality Leaders to observe demonstration of the patient safety standards and obtain feedback from patients and families regarding related care and identify areas of improvement. Organizes, promotes, and compiles campaign and education materials for annual quality and safety learning events, such as forums and workshops, to increase staff and patient/family awareness regarding patient safety.
  • Participates in the development and monitoring of quality initiative budgets within the context of operations demands and environmental and resource constraints and uses best practice methods to thoroughly monitor and adhere to allocated budgets. Coordinates all related payment to patient partners, supplies and participants of the quality initiative.

Qualifications

  • A level of education, training and experience equivalent to a Bachelor’s degree plus five (5) years recent related experience or equivalent work-related experience in a health, social services, government, or community organization.
  • Knowledge of research, quality improvement and clinical practices relevant to the service/program area; health systems and policy; and knowledge exchange. Understanding of and practical skills in quality initiative coordination/management, research and evaluation. Demonstrated ability to use relevant computer software applications including Microsoft Office, such as Word, Excel, Powerpoint, Outlook, and Sharepoint at an advanced level, and internet search engines and websites. Knowledge of funding structures and application processes, training and education materials and documents, and policy research. Demonstrated facilitation, sound negotiation, conflict management and consensus-building skills. Strong oral and written communications skills. Related experience in conducting literature searches and reviews, best practice reviews, policy analysis, and working collaboratively with a wide range of internal and external stakeholder groups. Demonstrated ability to work effectively and collaboratively in a demanding and dynamic environment. Qualities including: creativity, innovation, sound judgment, tact and diplomacy.

Job Type: Temporary, Full-Time - Until return of incumbent

Salary Range: $62,239 - $89,469 /year. The starting salary for this position would be determined with consideration of the successful candidate’s relevant education and experience, and would be in alignment with the provincial compensation reference plan. Salary will be prorated accordingly for part time roles.
